This is the process flow for rejection analysis

1. Team Formation  -  Create a team with Production, Quality, and Required persons

2. 5W2H  - Details of rejection should be filled in this format

3. Process Flow  - Where defective parts are produced in a flow ( Stations)

4. Containment action for Produced parts - Check all stocks in the company and  included customer ends stock if required

5. 4M Analysis - Man, Machine, Method, Material - Check which "M" got deviated from Specification

6.  Root cause analysis  - WHY WHY analysis  - Find the root cause for deviated process

7.  Reproduce the failure - Once you found the root cause, revalidate and confirm the deviation

8.  Action Plan - Take a permanent action plan to avoid the root cause

9. Monitor the result - A minimum of 3 months need to monitor the rejection trend

10. Sign off the process with solution and Result - If the result is found Ok, You can close the issue as solved.
